Southern California film students (high school [ages 14+ with guardian consent], undergrad and grad students [including students who graduated within the last 5 years] are invited to submit 30- and 60-seconds PSAs for BREATHE LA. The length (30- or 60-seconds) is the film maker’s choice.

Collaborations between Health Science and Film students are strongly encouraged.

BACKGROUND INFORMATION:

What is a PSA? A public service announcement (PSA) or community service announcement (CSA) is a non-commercial advertisement broadcast on radio or television, ostensibly for the public good. PSAs are intended to modify public attitudes by raising awareness about specific issues.

Through the MySpace and Facebook profiles, send us your name, email address, mailing address, phone number and the name of your school. If you are a finalist, you will be contacted.

2. PSA content:

This PSA's message: What a kid should do during an asthma attack (see the attached brochure).

Winning PSAs will incorporate the BREATHE LA logo, Asthma Lung Power information and note our website (www.breathela.org), and will be factually accurate.

3. Submit your PSA: Go to www.youtube.com/group/BreatheLAContest2008. If it’s not offensive, we will make it viewable to the general YouTube public. Submit by 11:59:59 PM on July 4, 2008.

4. Promote it: Get your friends and family to view your submission. Sure, it needs to have incorporated specific content criteria, but tie breakers are determined by viewing volume.

Prizes:
1st place prize: $1000 check.

PSA will be shown on BREATHE LA’s website.
PSA will be featured on BREATHE LA's YouTube page and could also be slected to be featured on YouTube's Nonprofit & Activisim page.

2nd place prize: $500 check.

3rd place prize: $250 check.

Breathe California of Los Angeles County (BREATHE LA) PSA Contest 2008 OFFICIAL RULES

1. Sponsor:

Breathe California of Los Angeles County (BREATHE LA).

2. Description of the contest:

This contest is a skill promotion in which you are invited to create an original Public Service Announcement (PSA) that informs youth and their guardians of what a kid should do in the event of an asthma attack. To do so create an original 30 or 60 seconds PSA that includes all criteria aforementioned (see “2. PSA message” above). Entries that are offensive to any person or group, or that contain any words or graphics that are lewd or pornographic, or that are inconsistent with, or disparaging of, Sponsor will be disqualified.

3. Entry:

This contest may be entered in one way during the period commencing at 12:00:01 AM, April 1, 2008 and ending 11:59:59 PM July 4, 2008. For purposes of these Official Rules (“Rules”), all times and dates are Pacific Standard Time.

Connect with us: Add BREATHE LA to your MySpace (myspace.com/bclac) or Facebook (search “BREATHE LA”) and to your YouTube account (www.youtube.com/groups/BreatheLAContest2008 ).

Submit: Upload your submission through our contest page at www.youtube.com/groups/BreatheLAContest2008 . Once approved, it will be posted for the general YouTube public to view.

4. Limitations:

All entries must be received by 11:59:59 PM July 4, 2008. Only one 30- or 60-seconds PSA entry per film maker is permitted. In event of a dispute regarding the identity of the person submitting an Internet entry, the entry will be deemed to be submitted by the person in whose name the Address is registered. Entries by any method other than set forth are void. Open only to Southern California high school students, college or university undergraduates, college or university graduate students and former college or university students having graduated between 2002-2007 living in the continental U.S. (excluding Breathe California of Los Angeles County [BREATHE LA] employees, Board of Director members [sitting and former] of BREATHE LA; relatives of BREATHE LA employees and Board of Directors members); their respective parent, subsidiary or affiliated companies; or the advertising, promotional or fulfillment agencies of any of them (individually and collectively, “Entities”); nor members of their households or immediate families. Neither entities, nor any of their officers, directors, shareholders, employees, agents or representatives (individually and collectively, “Releasees”) are responsible for Entries from persons residing, or physically located outside the continental U.S; Entries that are altered, delayed, deleted, destroyed, fraudulent, improperly accessed, inaccurate, incomplete, interrupted, late, lost, misrouted, non-delivered, stolen, tampered with, unauthorized or unintelligible; or any printing, production, technical, electronic or other errors; or for lost, interrupted or unavailable network, server or other connections; miscommunications; failed phone, computer hardware or software or telephone transmissions; technical failures; unauthorized human intervention; traffic congestion; garbled or jumbled transmissions; undeliverable e-mails resulting from any form of active or passive e-mail filtering; insufficient space in entrant’s e-mail account to receive e-mail; or other errors of any kind, whether due to electronic, mechanical or human error or other causes; even if caused by the negligence of any of the Releasees. Each of such potential Entries will be disqualified. Void where prohibited or restricted by law and subject to all applicable federal, state, local and municipal laws and regulations.

5. Ownership of Materials:

All Entries must be your original work, must not infringe upon third party rights, nor previously have won awards, nor been published; and you must own the PSA. If you are declared a Finalist, Sponsor, at its sole discretion, may use your PSA without additional compensation to you.

6. Prizes:

One (1) 1st Place winner prize: a $1000.00 (US Dollars) check and winning PSA shown on BREATHE LA’s website (www.breathela.org) and featured on BREATHE LA’s YouTube page (www.youtube.com/BreatheCaliforniaLA); one (1) 2nd Place winner prize: a $500.00 (US Dollars) check; one (1) 3rd Place winner prize: a $250.00 (US Dollars) check. Limit one (1) prize per person/family/household. All prizes will be awarded provided a sufficient number of eligible Entries are received and prizes validly claimed by July 31, 2008, after which no unclaimed prizes awarded.

7. Judging:

All Entries will be judged by Sponsor’s representatives on or about July 31, 2008, according to the following criteria: communication of message (60%), creativity (30%), and popularity (10%). A numerical score will be calculated using 100 as the highest number. The contestants with the six (6) highest total scores will be declared finalists (“Finalists”). The Finalists with the highest total score will be declared the winner of the First Prize and the Second and Third Prizes will be awarded to the other Finalists in the order of their scores. In the event of a tie, the contestant with the highest score for popularity will be declared a winner.

8. Winners:

The potential winners will be notified on or about July 31, 2008 by email, phone and US mail. Winners must be 16 years of age or older. Disqualification and the selection of an alternate winner may result from any of the following: [1] potential winner’s failure to respond to notice within seven (7) business days from the date of tirtiary contact; [2] the return of an email notification as undeliverable after three (3) attempts; [3] the return of any other notification or any prize as undeliverable; [4] potential 1st Prize winner’s failure to execute and return an Affidavit of Eligibility/Liability/Publicity Release within seven (7) business days from the date of tertiary contact; [5] potential 1st Prize winner’s failure to validly claim any prize by July 31, 2008; [6] potential winner’s failure to provide Sponsor with satisfactory proof that (s)he is the authorized account holder of the Address associated with a winning Internet entry; [7] potential Second or Third Prize winner’s failure to claim any price within (10) days after notice is sent and [8] any other non-compliance with Rules. All taxes are solely the responsibility of each winner; all winners will receive an IRS 1099 reflecting the final actual value of the prize. For winner’s list (after August 31, 2008) and/or Rules, send a self-addressed, stamped envelope to: BREATHE LA, 5858 Wilshire Blvd., Ste. 300, Los Angeles, CA 90036, Attn: BREATHE LA Contest 2008. 9. General Conditions:

Each contestant by entering this Contest agrees that [1] (s)he will abide by and be bound by the Rules and Sponsor’s decisions; [2] only the 1st Prize winning Entry will be shown on the BREATHE LA website with 2nd and 3rd Prize winning Entries acknowledged at the discretion of the Sponsor; [3] (s)he hereby irrevocably assigns and transfers to Sponsor and its assigns all rights, title and interest in the PSA, including without limitation copyrights, and agrees to sign all documents necessary to effect such transfer; [4] (s)he hereby grants to Sponsor and its assigns the right to publicize, use, adapt, edit and/or modify such PSA in any way and in any and all media, without limitation, and without consideration to contestant; [5] the Releasees are not responsible for claims, injuries, losses or damages of any kind resulting, in whole or part, directly or indirectly, from the awarding, delivery, acceptance, use, misuse, possession, loss or misdirection of any prize; participation in this Contest or in any activity or travel related thereto or from interaction with, or downloading of, computer Contest information; [6] winner’s acceptance of any prize constitutes the grant of an unconditional right to use winner’ name, address, (city and state only), photograph, biographical and prize information and/or statements about the promotion for any publicity, advertising and promotional purposes without additional compensation, except where prohibited by law; [7] in the event of viruses, bugs, unauthorized human intervention, Acts of God, acts or regulations of any governmental or supra-national authority, war, national emergency, accident, fire, riot, strikes, lock-outs, industrial disputes, acts of terrorism, or other matters beyond the Sponsor’s reasonable control, corrupt, prevent or impair the administration, security, fairness or proper execution of this Contest, so that it cannot be conducted as originally planned, the Sponsor has the right to cancel, terminate or suspend the Contest; and in such event, to select winner by such method as Sponsor in its sole discretion shall consider equitable; [8] the Releasees are not responsible for typographical or other errors in the offer or administration of the Contest, including but not limited to: errors in the advertising, Rules, selection and announcement of the winners and distribution of the prizes; [9] any portion of any prize not accepted or used by any winner will be forfeited; [10] the Releasees are not responsible for any inability of any winner to accept or use any prize (or any portion thereof) for any reason; [11] Sponsor has the right to disqualify any individual found to be: (a) tampering or attempting to tamper with the entry process or the operation of the Contest of the Website (b) violating the Rules; (c) violating the terms of service, conditions of use and/or general rules or guidelines of any Contest property or service or (d) acting in an unsportsmanlike or disruptive manner, or with intent to annoy, abuse, threaten or harass any other person; [12] Sponsor has the right to lock out a contestant whose eligibility is in question or who has been disqualified or is otherwise ineligible to enter the Contest; [13] since any attempt by a contestant to deliberately alter or damage the Website or undermine the legitimate operation of the Contest is a violation of criminal and civil laws, Sponsor shall have their right to seek damages from any such contestant to the fullest extent permitted by law; [14] the Releasees are not responsible to the cancellation of the Contest or a change of date; [15] any dispute that arises regarding the meaning or interpretation of the Rules shall be resolved by applying the laws of California, without regard to conflict of laws provisions therein, and shall be solely and exclusively brought in the state or federal courts located within that state.
